Health Services

September 2015

Dear Parents,

Hello! My name is Cathy Glisson and I am the school nurse at Met East High School.
Please feel free to contact me with any concerns regarding your child's health and welfare. My office phone number is 966-5223, press #4 or enter ext. 31432.

Please have a current and working telephone number in the school office in the event your child needs your attention. If your child becomes ill or injured during school hours and must go home, either you or someone listed on your form will be contacted for pick up. We must have working numbers to avoid any unnecessary delays in caring for your child.

Keep your child home if they are sick. A fever (100 degrees), vomiting, diarrhea, excessive coughing, profuse nasal drainage, unexplained rash, severe sore throat, persistent headache, persistent earache, injury to an extremity and unable to bear weight are all reasons to stay home. Your child must be fever free for 24 hours before returning to school.

A great website to visit – Academy of Pediatrics: healthychildren.org

Please fill out and return to the school nurse the attached forms:

• Health Information Form
• Emergency Form
• Consent to Share Information

I look forward to you working with you all during this school year!

Sincerely,

Cathy Glisson BSN, RN, CSN
